Getting started

How Getting Support Works

Whether you are looking for help for yourself or for someone you care about, we take it at your pace and guide you through every step.

Step by step

Our Simple Intake Process

  1. 1

    Reach out

Call us or send a request. We listen to what is going on and what you hope will change.

  2. 2

    Meet with us

We meet you at home, or somewhere you feel comfortable, to understand your needs, routines and goals [confirm].

  3. 3

    Your support plan

Together we agree a personal plan: what support, how often, and the goals we will work towards.

  4. 4

    Support begins

Your support worker starts visiting, and we review the plan with you regularly.

Before we start

What Helps Us Prepare

A little information up front helps us support you well from the very first visit.

  • Your goals. What would you like to be easier, or to be able to do again?
  • Your routines. The times of day that work best, and anything that helps you feel at ease.
  • Your care team. Contacts for your doctor, therapist or case manager, if you have them [confirm].
  • Paperwork. [List any intake or referral documents you require].
